Question on using MilkSync for BB AND for Outlook

With that new tool, I would to synchronize RTM with Outlook, and with my BB directly, and then stop synchronizing my BB with Outlook. That will make RTM central:

Outlook <-> RTM <-> BB

Is that supported, or is there also a risk of lost/duplicate entries?

That should work fine, as long as you disable any synchronization between your BlackBerry and Outlook (including any wired syncing or automatic backups). The only problem arises when you have items synchronizing in a "circle", as any changes can be passed back and forth.
